  • Please complete the following questionnaire with honest responses to assist us in assessing whether you may be eligible for WBE, MBE, and/or DBE certification. This is a preliminary pre-qualification tool that will guide us in determining the appropriate next steps on your journey toward Certification, and how we can best support you in achieving your objectives.

    While completing this questionnaire and engaging in any offerings from HIT Executive Consulting does not guarantee Certification, we provide the optimal path to success, assuming your company meets the qualifications!

  • 1. Has your company previously been denied certification from any government or third party certifier ever?*
  • 2. Is the Company to be Certified a For-Profit company?*
  • 3. Is your business formation and principal place of business located in the US or its territories?*
  • 4. Is the Qualifying Individual a US Citizen or Lawful Permanent Resident (LPR or "Green Card" holder)? NOTE: ALL U.S. based Certifications require the Qualifying Individual(s) to be a US Citizen and/or a LPR*
  • 5. Is your company’s Management and Daily Operation controlled by a Qualifying Individual(s) with Industry Experience?*
  • 6. Is/are the qualifying individual(s) a woman or women?*
  • 7. Is/are the qualifying individual(s) a recognized minority (Asian American, African American, Hispanic American, or Native American)?*
  • 8. Does the Qualifying Individual work at least 30 hours a week in the applicant company, during normal hours of operation? NOTE: This may not be 9-5 or "normal" work hours.*
  • 9. Is the Qualifying Individual(s) personal net worth under $2.047 Million dollars, not including the applicant company, Primary Residence, Retirement Investments (If you are WAY OVER the $2M limit, then "No").*
