Permission to Ride

At Camp Wakonda, sometimes we need to take the staff off campus, whether it be for group activity or a couple staff doing a quick "Walmart Run". We want to give you, as the parent/guardian, the opportunity to let us know what you are most comfortable with.

*To be clear, this form only refers to staff taking other staff off campus. If your minor is leaving campus with anyone besides a parent/guardian for personal reasons, it will require a phone call, from you to the camp office.*

I hereby grant permission for my child (under the age of 18) to ride in a vehicle with the following Camp Wakonda staff member(s) in personal vehicles during time off, for group recreation, or for camp assigned work activities.
