MEMBERSHIP APPLICATION

Membership Applications shall be voted on by the CVFSC Board of Directors, each new member will be notified by the Membership Chair.

Volunteer hours are mandatory for parents of CVFSC club skaters.

Parents of skaters are considered "Covered Individuals" according to USFSA and will be required to complete the Safe Sport training and background check. Therefore in order for club members under 18 years of age to be in good standing with the CVFSC, one parent will need to successfully complete the Safe Sport Training and background check.

In order for adult club members to be in good standing with the CVFSC, they will need to successfully complete the Safe Sport Training and background check for themselves. USFSA allows 45 days from the starting date of membership (July 1st) to become compliant.

CVFSC board members are asking that you complete your Safe Sport Training by August 15, 2023. Club ice will be withdrawn after this date until Safe Sport Training is complete.

CODE OF CONDUCT/ETHICS

Each member of the Cache Valley Figure Skating Club is responsible to know and adhere to the rules, regulations, and codes of conduct and ethics of the CVFSC,the USFSA and if a coach, the PSA, as well as the Sportsmanship and Freestyle Session rules of the Eccles Ice Center and any other facility at which the club may be competing, performing, or participating in any program. Failure to adhere to the code of conduct, code of ethics, or other rules set out by the club, USFSA or PSA, consequences will result based on the policies in place by those organizations and their boards.

As a member of the Cache Valley Figure Skating Club, I agree with the information listed in this membership packet, code of ethics and conduct, and the CVFSC Bylaws, and that I must uphold these codes. Failure to adhere to the code of conduct, code of ethics, or other rules set out by the club, USFSA, USOC SafeSport or PSA, consequences will result in actions based on the policies in place by those organizations and their boards.

I also agree to read and abide by the Freestyle Session Rules and Regulations as outlined by the Eccles Ice Center when participating in that rink’s sessions, and to register and sign an annual release of liability for use of the Eccles Ice Center facilities.

MEDIA RELEASE
As a member of the CVFSC, I do hereby grant the Cache Valley Figure Skating Club the right to use my name, photograph and/or image for club sponsored activities, news, and promotions, including but not limited to ice show videotaping, the club bulletin board and club website.I would like the right to omit my (or my skater’s) name and/or photograph from any and all publications if I choose to, on an event-by-event basis. I understand that if I choose to exercise this option, I must contact the CVFSC Board in writing in advance of the event so that arrangements can be made before filming and/or publishing takes place.
