    Please complete all fields. Staff will review your initial request. Once the location and size of the tap are approved you will be quoted for the tap fee. Payment must be received before the tap will be installed. Customer must set up an account before a new tap can be made. If you do not already have an address assigned to the tap location you will need to visit jackson-ms.viewpointcloud.com to have a new address assigned. Please allow up to ten (10) Business Days for initial review. Once approved you will receive a schedule for the tap installation. Proposed tap locations must be physically marked by stake or paint. Failure to mark proposed tap location will result in delayed review.

    "Subscriber" in consideration of being supplied with water by the CITY SERVICES, CITY OF JACKSON, MISSISSIPPI, hereafter called "Municipality", at the stated location or any other location requested, as hereinafter set forth, agrees to pay all bills rendered under prevailing rates for water and/or sewer services, hereafter referred to as "services", as established by City Ordinance. Municipality will supply, as hereinafter provided, to subscriber services and subscriber must provide, free of charge to Municipality, a place satisfactory to Municipality for a meter; subscriber will protect meter at all times and guarantee its return in good condition; subscriber will file with Municipality a deposit as continuing security for the performance of all obligations, including the discharge of all debts for services rendered until notice is given by subscriber for discontinuance of said services. Municipality will use all reasonable diligence within its power to provide uninterrupted services, but does not guarantee a constant supply, nor the quality, nor suitability of the water, and will not be liable for any damages to persons or property caused either by a failure in constant supply, or by unsuitableness, or quality. Municipality reserves the right to cancel this application and contract, and to refund the deposit if the cost to the Municipality of making connection should be excessive. Failure by subscriber to promptly pay bills for services rendered by Municipality, under this contract at the stated location, or elsewhere in the service district of the City of Jackson, shall terminate and cancel all of the subscriber's rights hereunder, and Municipality may discontinue its service and remove its meter. In the event it becomes necessary to discontinue services for nonpayment of any service rendered under this application, subscriber agrees to pay Municipality the service charges of disconnecting and connecting meter upon reinstatement of services as set forth by City Ordinance. Subscriber shall pay for all services until notice is given by subscriber for discontinuance of services, and until proper time allowed for Municipality to thereafter finally terminate the services by removal of the meter. Failure by subscriber to apprise Municipality of changes of mailing address may result in discontinuance of services by removal of the meter. Reference is hereby made to the Ordinance of the City of Jackson relative to water, sewer and sewer disposal as if copies in full herewith. This contract contains all the terms of the agreement and no oral understandings are binding.
