I attest that the following is accurate:
- I completed this application with accurate, honest, and complete information. I acknowledge that failure to provide the SAC with accurate, honest, and complete information will delay or possibly deny my request to move down one level from the division in which I last played.
- I understand that if reassessment is required due to inaccuracies or omissions in this form, reassessment is at my own cost.
- I understand that the SAC may require me to attend a formal on-ice skills assessment should a decision not be reached based on the information that I provide in this application.
- I understand that WHAM teams are only allowed to roster two (2) players per season who have dropped down that season.
- I understand that if my request to move down is approved, the following conditions will apply:
- I will not be allowed to roster as a secondary/sub player in the division from which I'm moving down. For example, if I am moving from B1 to B2, I understand that I cannot register as a secondary/sub player in the B1 division.
- I will be on probation for the upcoming two (2) seasons, during which time the WHAM board and/or SAC can move me back up to my original division if it is determined that my level of play exceeds that of the lower division.
