Parental Consent

I hereby consent to let my child participate in Making Excellence Possible, Inc. (MEP) Tutoring Services! It is understood that every precaution will be taken for the safety and well-being of my child, but in the event of accident or sickness, Making Excellence Possible, Inc., its staff and its contractors, volunteers are hereby released from any liability. It is understood that failure to follow the program guidelines for parent involvement will result in dismissal from the program.

I grant permission for my child to be photographed or recorded during program activities.
