Membership Cancellation Application

All members wishing to cancel their memberships must do so by WRITTEN NOTICE. Submitting this application, Fitness Nation confirms your intention to cancel the membership satisfying the contractual written notice requirement.

This form however is just an application and the first step in the process of your cancellation. We require your information to confirm your profile and to ensure you have met all contractual requirements as stipulated in your membership agreement, this process may take some time so we appreciate your patience.

We request that the cancelling member organises to meet with a member of our staff over the next couple of days where our staff can finalise all contractual requirements or ensure that they are in good order to proceed with the official CANCELLATION of the membership.

If you are out of contract, your membership will be cancelled 30 days after submitting this form.
If you are still in contract, you will be subject to a cancellation fee, and a member of the sales team will contact you to discuss your options.

Once you have submitted your cancellation request, we will process it as quickly as possible. You will receive a confirmation email once your cancellation is complete.
