This form is for any new clients (or clients I have not seen within 6 months) wanting to schedule a color service. The color services I offer are blonde or brunette coloring. I do not offer vivid colors, red, or on scalp lightening services. This form will determine whether your service request and personality aligns with what I offer, to ensure you have the best possible experience.
