This application is to be filled out only if you receive one of the following government benefits:
- SNAP Food Stamps
- SSI Disability
- WIC
- Medicaid
- Section 8 Housing
- Social Security* (*62 years of age or older and it's your only source of income)
PLEASE NOTE - you must provide proof of receipt of one of these benefits to participate in the Food Pantry Program on an ongoing basis by uploading a copy of a document below.

*Due to extremely high demand for this program, we can only provide a maximum of two bags of food per household per month as follows:
- One bag each of cat and dog food or
- Two bags of cat food or
- Two bags of dog food
