Hi parents! Please complete the following form PER COSTUME/NUMBER. Not per child! If you don't mind, please just submit 1 form for each duet, trio, etc. We will post the charges to the respective accounts, not just to the parent who submitted the form. Thank you!

Have you referred to the list of current costumes being used (misskelleys.com/solos) and are ready with NEW options? (We are working on building a list for the past 5 years, though this is not ready yet. So please understand that one or more selections may still be rejected due to having been used recently. But we hope the current list will be helpful to you!)*

Do you understand that bra tops are not allowed? You may check the description if in doubt if it is a crop or bra.*

Do you understand that clearance, “limited edition”, or “rev rack” costumes can be SEVERELY limited in available sizes? You are more than welcome to include some clearance options, but please be sure to include non sale options as well!*

Do you understand that you MUST include 4 DIFFERENT costumes? The same one or two may not be listed multiple times.*

I understand that choices are limited and I MUST provide options!
Do you understand that only the approved costume may be worn? Should any changes arise, the new costume must also be approved.*

Have you checked each company’s size chart (misskelleys.com/solos), measured your dancer and are prepared to choose your own size? AND have you checked what sizes your costume choice is actually available in? (Not all costumes are available in all sizes!)*

Do you understand that ship times vary from costume to costume, and that the studio has no control over ship times or any delays in shipping?*
