I understand that attendance of and Evaluation is not without risk and I hereby waive and release D&C DOG TRAINING, LLC of Woodlawn, TN herein after referred to as the “Training Organization”, and its officers, members and agents from any and all liability of any nature, for injury or damage which I, or my dog may suffer, including specifically, but without limitation, any injury or damages resulting from the action of any dog. I expressly assume the risk of such damage or injury while attending any training, or any other function of the Training Organization, or while on the grounds or on the surrounding area, thereto. In consideration of and as inducement to the acceptance of my application for training, I hereby agree to indemnify and hold harmless this Training Organization from any and all claims as a result of any action by any dog, including my own.

Reservation fee is nonrefundable and will be forwarded to agreed training plan upon showing up to the scheduled time slot. If at anytime the applicant decides to cancel their reservation spot without a 24 hour minimum notice and/or is a no-show to their scheduled time slot, then there is a cancellation fee of $25 that is non-negotionable and non-refundable.
