Mosaic's 2026 Women of Vision Nomination Form

Please complete this application to nominate for an award.

Mosaic of North Central Wisconsin invites you to nominate a woman who has provided inspiring, innovative, impactful leadership and/or made significant contributions to communities.

Nomination Deadline: March 15th, 2026. ALL MATERIAL MUST BE SUBMITTED BY 5:00 p.m. on this date. LATE APPLICATIONS WILL NOT BE CONSIDERED.

Recognition Luncheon: May TBD, 2026. Registration begins at 11:00 a.m., lunch and program are held from 11:20 a.m. to 1:00 p.m. Honorees will be selected by April 1st. There will be a short gathering event the before the event to go over any questions and meet past WOV honorees. If chosen as an honoree, the nominee must attend the luncheon. Honorees will need to attend a short, informational gathering prior to the event. Date and time will be based on availability.

Nomination Criteria - explain how and to what extent the Nominee:
1. Is a motivated woman with innovative ideas aimed at fulfilling community needs. (10 pts) - Describe how the nominee’s vision or pioneering strength and spirit have led to outstanding contributions.

2. Has made contributions that benefit the community. (10 pts) - Describe the project(s) for which the candidate is being nominated.

3. Has initiated or played a leadership role in a project that has had significant impact. (8 pts) - Describe who has benefitted from the projects described above and how they have benefitted.

4. Has served as a role model or mentor for others professionally, personally, and/or through community service. (7 pts) - Describe how the nominee has served as a role model or mentor for others and is a daily example of inspiration. Please provide a brief review of the nominee’s education background and career biography.

5. Is involved in multiple areas of the community. (5 pts) -Please list any professional, civic, or community involvement, including honors and awards received.
