Billabong Zoo Keeper for a Day Consent

The following must be read and accepted by a parent or legal guardian prior to attendance. By agreeing to this consent form, you are allowing your child to participate in Zoo Keeper for a Day at Billabong Zoo.

Realising that every care will be exercised, I hereby indemnify Billabong Koala and Wildlife Park Pty ltd, its owner and staff against all actions, suits, claims and demands (including costs) for personal injury or damage to, or loss of, personal property incurred as a result of my child’s attendance at this program.
In the case of accident or emergency, I give my permission for the Billabong Koala and Wildlife Park Pty Ltd staff to seek medical treatment by a medical practitioner, hospital or ambulance service. I agree to bear any costs thereby incurred.
The information I have provided in this booking is true and accurate.

Junior Zoo Keepers

A limited edition Billabong Zoo hat will be provided at the beginning of the day.

Teen Zoo Keepers

A limited edition Billabong Zoo tshirt will be provided at the beginning of the day.

Permission to Publish

During Zoo Keeper for a Day programmes there may be occasions where staff may photograph, film or record students participating in Billabong Zoo activities.
We do this for many reasons; including to celebrate student participation and achievement, showcase particular learning programs, communicate on

webpage, our social media sites. This notice applies to photographs, video or recordings of students that are collected, used and disclosed by Billabong Zoo. Billabong Koala Wildlife park staff photographing, filming or recording students will always do so in a respectful, appropriate and safe manner.

We ask for your consent to photograph, video or record your child during the activity of Zoo Keeper for a Day that may be used on our website or our social media platforms only.
