Welcome to the Ethos Youth Ensembles!  Whether this is your child's first time playing (Academy 1) or if they have played before (Academy 2, Reading Orchestra, Camerata Orchestra, Chamber Orchestra, Jazz Ensemble, or Wind Symphony), we are proud that Ethos will play an important role in your music education!

This form should be completed by parents, not students, and will take approximately 10 minutes to complete.  We will collect signatures for legal forms (such as photography/video release), medical information in case of emergency, and will collect tuition as well.

Please have your medical insurance card, pediatrician's contact information, and credit card (if you choose to pay via credit card) before beginning.

Consent and Agreement

In the event that {studentName} is presented for, or requires medical treatment or surgery or any other form of medical care or aid, I, {primaryParentguardian26}, as the parent/legal guardian of the above named student, do hereby authorize the Directors/Chaperones standing in loco parentis to be consulted with, and consent to, any medical treatment or care deemed necessary by any doctor, nurse or other medical personnel. I also guarantee payment of all charges incurred for medical treatment such as, but not limited to physician, hospital, x-ray, lab, drugs, and EMS. (For emergency use only. Every effort will be made to contact parent/guardian prior to treatment). I agree to indemnify and hold harmless the faculty, board and chaperones of the Ethos Youth Ensembles, First United Methodist Church in Murfreesboro, and Rutherford County Schools, its agents and employees for any and all claims, demands, actions, rights of actions and/or judgments by or on behalf of the above named student arising from or on account of said procedures and/or treatment(s) rendered in good faith.

I acknowledge the contagious nature of the novel coronavirus (COVID-19) and other contagious pathogens. I further acknowledge that the Ethos Youth Ensembles cannot guarantee that my student(s) will not become infected with COVID-19 and any other contagious pathogens while onsite and/or while participating in performing arts activities.

I understand that the risk of becoming exposed to and/or infected by COVID-19 and other contagions pathogens may result from the actions, omissions, or negligence of myself, my family members, my student(s) and others, including, but not limited to, Ethos Youth Ensembles teachers, staff and administration.

I voluntarily seek the educational services provided by the Ethos Youth Ensembles for the 2023-2024 academic year (July 1, 2023 through July 31, 2024), and acknowledge that it is possible that I may be increasing my risk, and/or that of my family members and student(s), to COVID19 and other contagious pathogens.

I acknowledge that I, and my student(s), must comply with all procedures set by the Ethos Youth Ensembles to reduce the potential spread of COVID-19 and other contagious pathogens while onsite and/or while participating in Ethos Youth Ensembles activities.

I hereby release and agree to hold the Ethos Youth Ensembles harmless from, and waive on behalf of myself, my student(s), my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act by the Ethos Youth Ensembles.

I understand that this release discharges the Ethos Youth Ensembles from any liability or claim that I, my student(s), my heirs, or any personal representatives may have against the Ethos Youth Ensembles with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any educational services received from the Ethos Youth Ensembles.

By entering my name below, I attest that I, {primaryParentguardian26}, am a parent or legal guardian of {studentName} and I agree to the terms of the Contagious Pathogen Waiver.

I hereby authorize the Ethos Youth Ensembles, hereafter referred to as “Company,” to publish photographs taken on between July 1, 2023 and July 31, 2024 of myself and/or the minor child or children listed below, and our names and likenesses, for use in the Company’s print, online and videobased marketing materials, as well as other Company publications.

I hereby release and hold harmless the Company from any reasonable expectation of privacy or confidentiality for myself and for the minor child and children listed below associated with the images specified above. Further, I attest that I am the parent or legal guardian of the child or children listed below and that I have full authority to consent and authorize (insert company name) to use their likenesses and names.

I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other Company publications. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.

I hereby release the Ethos Youth Ensembles, its contractors, its employees and any third parties involved in the creation or publication of Company publications, from liability for any claims by me or any third party in connection with my participation or the participation of the minor children listed below.

The Ethos Youth Ensembles function on a high level due to the coordinated efforts of Ethos Administration, Faculty, Parents, and community partners that host Ethos rehearsals, classes, and performances; we cannot be successful without everyone present at all Ethos events.  Be proactive in maintaining a personal calendar with your dates for family, church, sports, Ethos, etc.  Communicate any potential conflicts that might arise to your Ethos teacher.

Please familiarize yourself with the Ethos Calendar (found at www.EthosMusic.net) and ensemble-specific/class-specific calendars handed out by Ethos teachers, which include specifics on call times, how performances are graded, and alternative assignments to missed performances.  Note that all performances are ticketed events, with admission open at costs that are in addition to Ethos tuition.

I understand that a comprehensive Ethos performance calendar is available at www.EthosMusic.net and understand that class-specific/ensemble-specific calendar dates will be provided by Ethos teachers.  I understand that all performances are ticketed events, with admission open at costs that are in addition to Ethos tuition.

I have read and understand the policies and procedures outlined in the Ethos Parent/Student Handbook, found at www.EthosMusic.net.

By entering my name below, I attest that I, {primaryParentguardian26}, am a parent or legal guardian of {studentName} and I agree to the terms of the Handbook and Policies Signature Form.

Tuition for each Ethos class/ensemble can be found on the "Tuition" tab of www.EthosMusic.net.  

Families have three options to pay your Ethos Tuition:

1. Via check in the Full Amount, paid before your first class.  Ethos will not accept or process checks for partial amounts.  Please mail your check to Ethos Youth Ensembles, 1316 Stewart Creek Road, Murfreesboro, TN 37129 prior to your first class/rehearsal.
2. Via Credit Card for the full amount, paid before your first class.
3. Via Credit Card in monthly installments.

Audition Sign Ups

If you are signing up for Academy 1, Academy 2, or Reading Orchestra, no audition is necessary.

If you are auditioning for Camerata Orchestra, Chamber Orchestra, Wind Symphony, or Jazz Ensemble, you MUST sign up for an audition time at www.EthosMusic.net under the Auditions tab (audition slots will go live 2-3 weeks before the audition dates).
