I hereby voluntarily permit my child to participate in the Ness Skills and Drills Workouts. I understand and fully accept that there are risks involved in sports, and that accidents and injuries are common and are ordinary occurrences of sports. I hereby agree to accept any and all risks of injury or death.
As consideration for being permitted by Ness Skills and Drills to participate in these activities, I hereby release and hold harmless Ness Skills and Drills, its staff, volunteers, designated coaches from all liability, and from all actions or claims that my child now or hereafter have for damage or injury to my child, or to any person or property, resulting from the negligence or other acts of any employees or volunteers in connection with my child’s participation. I further agree that this waiver, release and assumption of risks is to be binding on the heirs and assigns of the undersigned. I further agree to indemnify and to hold Ness Skills and Drills (its officers, employees, agents and volunteers) free and harmless from any loss, liability, damage, cost or expense which they may incur as a result of any injury and/or property damage that my child may cause or sustain while participating in this activity.
I have carefully read this release and fully understand its contents. I am aware that this is a release of liability and a contract between me and ness skills and drills and sign it of my own free will.
RELEASE OF MEDIA FORM
I hereby grant Ness Skills and Drills LLC and their agents, the absolute right and permission to use photographic portraits, pictures, or digital images of My Child, or in which My Child may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any sports publication, social media, or website of any entity within the Ness Skills and Drills LLC building, without payment or any other consideration. I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein My Child’s likeness appears, or the use to which it may be applied.
I hereby release, discharge, and agree to indemnify and hold harmless Ness Skills and Drills LLC, or any agent/entity within Ness Skills and Drills LLC, from all claims, demands, and causes of action that I or My Child have or may have by reason of this authorization or use of My Child’s photographic portraits, pictures, or digital images, including any liability by virtue of any blurring,distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or in processing tending towards the completion of the finished product, including publication to the internet, in brochures, or any other advertisements or promotional materials.
