I, the undersigned, authorize and direct , and its successors or assigns, to deduct from my pay an amount equal to the dues and initiation fees in the amounts fixed in accordance with the Bylaws of Local Union 824 and the Constitution of the International Brotherhood of Electrical Workers and to pay same to said Local Union in accordance with the terms of the bargaining agreement between the Employer and the Union.

This authorization is voluntarily made in order to pay my fair share of the Union's cost of representing me for the purposes of collective bargaining, and this authorization is not conditioned on my present or future membership in the Union.

This authorization will continue until revoked by me through written notice to you and I agree that this authorization shall be automatically renewed and irrevocable  for successive periods of one year unless revoked by written notice to you within the thirty

(30) day period prior to the anniversary or expiration date of the Collective Bargaining Agreement between the Company and the Union.

I understand that under current law the payments covered by this authorization are not deductible as charitable contributions for federal income tax purposes.

I, the undersigned Employee of Embarq-Florida, Inc. authorize the Company to deduct from the first paycheck of each month those dues as certified as official dues by the Financial Secretary of Local 824, IBEW AFL-CIO. Also deduct $1.00 per Month as dues to the Fraternal Order of Telephone and Electrical Workers, Inc. and pay this amount to Local 824 of the International Brotherhood of Electrical Workers in accordance with the agreement between said Local Union and the Company. “Fees, dues and assessments covered by this authorization are not deductible as charitable contributions for federal income tax purposes." This authorization and direction is revocable but shall continue in force and effect until revoked by written notice to the Company and the Union
