  • WAXING CONTRAINDICATIONS: You may NOT be waxed if you are currently using or have done any of the following in the last 24-48 hours:

    • Accutane
    • Antibiotics
    • Blood thinners
    • Retinols
    • Sunbathing/Tanning
    • Sunburned
    • Consumed Alcohol
    • Have any open lesions/wounds, rashes, or general breakout on or near area wanting waxed
    • Face Waxing: Ingredients in Skincare Products: Retinols, Acids (salicylic, glycolic, AHA/BHA, Chemical Peels, etc.)

  • POLICY 

    GENERAL

    • No extra guest allowed in suite.
    • I do not offer intimate waxing and/or services for anyone under the age 18. For all other services for anyone under 18, you must have parent signature and consent in suite or parent present during services.
    • For intimate waxing services, I ask that you please arrive freshly cleaned so that the area is free of any oils, sweat, bacteria, or bodily fluids to ensure comfortability and a thorough & efficient wax service to both of our standards as well as safety and sanitation requirements.
    • Menstrual Waxing- You can be waxed while on your period as long as you have a freshly put in tampon (no exceptions) in order to maintain safety and sanitation requirements. I get she may come at the most inconvenient times! But if you are not comfortable waxing while on your period, please try to book around it as best as you can!

     

    DEPOSIT

    A deposit will be required at booking to secure appointment date & time and will go towards your service amount. Deposit is non-refundable with only the exception being if there was a mistake made on my end.

     

    LATE POLICY

    If you are running late, please notify me by text as I may be with a client at (864)735-0574. There is a 6 minute grace period after scheduled appointment time. SG Aesthetics does not extend service times, issue refunds, or reschedule same day appointments for late arrivals. After 6 minutes, your appointment is canceled and your deposit is forfeited. You must go online to reschedule your appointment with a new deposit.

     

    RESCHEDULE/CANCELLATION

    You get one “free” reschedule and must give notice of at least 48 hours of scheduled appointment time. Failure to do so within appropriate time frame results in deposit being forfeited and 50% fee of services charged to card on file.

     

    NO SHOW

    No shows are subject to be charged 85% of scheduled services and will have to pay 100% of future services at booking.

     

    REPEAT OFFENDERS

    Habitual offenders of any stated policy may be subject to having to pay 100% of all future services at time of booking and/or terminated as a client of SG Aesthetics.

  • Photo Consent

    Photos or videos taken by SG Aesthetics can be used for treatment progress, insurance purposes, marketing, potential advertising, promotional, or any other lawful purposes and will not be used for any other reasons than stated above. These photos do not have to include your face or any identity features. There is no timeframe on when these photos shall be used and they can be published in various media formats. SG Aesthetics does not share information about photos or clients themselves online or with any other person.

    By agreeing, you grant SG Aesthetics the right to use, distribute, and display my image and likeness, alone or in conjunction with other images, in any medium, for any purpose, without compensation to me. This right includes the rights to all uses listed above. I waive any right to approve or inspect the photographs or films and release SG Aesthetics LLC and it's affiliates from any and all claims arising out of the use of photos and films, including without limitation any claims for libel, invasion or privacy, or misappropriation of likeness.

    I understand that I have the right to revoke this release at any time, by providing written notice to SG Aesthetics. However, I acknowledge that revoking the release will not affect any use of of my image or likeness that occurred before the revocation.

  • Disclaimers: By signing and submitting, you agree to everything stated in policy list for SG Aesthetics and everything stated below:

     

    • I agree that all information in forms is true and accurate. I agree to keep the esthetician updated as to any changes in my medical profile prior to every session. I understand that there shall be no liability on the esthetician should I fail to inform the esthetician of any changes.
    • I also understand the risks and dangers of waxing such as any adverse reactions that may include but are not limited to: redness, itchiness, histamine reaction, or bumps.
    • If I experience any pain or discomfort during the session, I will immediately inform the esthetician so that the products and/or technique may be adjusted to my level of comfort or esthetician may choose to discontinue service.
    • I understand that estheticians are not qualified to perform, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given should be construed as such. Any information given to me is for educational purposes only. My esthetician can only advise me to seek medical help from a medical professional.
    • I understand that the services offered are not substitute for medical care, and any information provided by the therapist is for educational purposes only and not diagnostically prescriptive in future
    • I understand that any illicit or sexually suggestive remarks or advances made by me will result in immediate termination of the session. My esthetician also has the right to refuse or discontinue service under any circumstances to her discretion.

  • Liability Waiver

     

    I release SG Aesthetics LLC and esthetician, Sydney Garrett, from an responsibility and/or liability concerning the application, processing, and/or consequences of the beauty service(s) I selected to receive. I consent to have beauty services of my choice applied.

     

    I release and hold SG Aesthetics LLC, it's employees and it's agents harmless against any and all liability, damage, and/or expenses arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or physiological) that might incur as a result of the services provided today. I understand that additional treatments may be recommended and/or necessary for maintenance. This form in its entirety is in good standing for 3 months.
