Thank you for selecting Centre Place Dental as your dental health care provider.  We are committed to the successful treatment of all your dental needs.  The following is a statement of our Financial Policy, which we require that you read and sign prior to any treatment.

For your convenience we accept cash, check, Visa, MasterCard, and Discover. We also offer a payment plan through Care Credit (subject to credit approval) for patients who prefer a monthly payment. For patients who pay with cash at the time of service we offer a 3% bookkeeping courtesy on procedures of $500.00 or more. Centre Place Dental charges $25.00 for returned checks. Any balance(s) left unpaid after 30 days from the date of service will incur 16% interest per annum (1.33% per month). We also reserve the right to assess a $25.00 late fee per month for patient accounts that have not been paid in full in 30 days or have not called and made other acceptable arrangements.

While we accept many insurance benefits, we do require that your estimated co-pay and deductible be paid at the time of service. Benefit information is the responsibility of the card carrier even when Centre Place Dental files your insurance claim. The insurance policy is a contract between you and your insurance company. Centre Place Dental is not a party to that contract. Any portion your insurance company declines to pay for becomes your responsibility.

We reserve the right to assess a fee of $50.00 for patients who miss or cancel more than one time in a calendar year without 24-hour notice.

In signing this Financial Policy, you acknowledge that you understand and agree to comply with Centre Place Dental policies.
