Waivers - Release of Liability - Consent and Release Forms

The following section contains important information required to honor your commitment to Camp Rising Sun. Please read each section carefully and initial that you give your consent. If under 18, signature of Parent/Legal guardian will also be required.
Insurance Waiver

Campers and volunteers at Camp Rising Sun are not covered by accident insurance. It is advised that you make your own arrangements for adequate insurance coverage.

As a CIT, volunteer, or staff member of Camp Rising Sun, I do hereby absolve Camp Rising Sun, its sponsors, staff, directors, and participants from any injury arising out of participating in this camping program. I assume all risks and hazards in this outdoor recreational activity and agree to waive any claim for liability against Camp Rising Sun due to any injury associated with any camp activities. The undersigned is aware of potential risks and agrees that this waiver applies to traveling to and from camp, attending the camp and participating in any camp programs and events.

Medical Consent

I approve this application, and certify that I am capable of such an experience. I agree to notify the camp directors and medical staff if I or any member of my family are exposed to any communicable diseases during the three weeks prior to camp. I consent to the administration of first aid and routine care for me by camp staff, and that camp staff may seek appropriate emergency room treatment if necessary. I give my approval for the camp nurse to contact my physician if necessary.

Photo Release

I hereby authorize Camp Rising Sun to use pictures of me taken in a photograph, digital image, videotape, motion picture, and/or testimonial (written words). By signing this contract, I do hereby release and absolve Camp Rising Sun, its agents, sponsors, staff, directors, participants, as well as any and all users and exhibitors of said pictures, from any and all claims, demands, accountings, and causes for which the aforesaid photograph, digital image, videotape, motion picture, and/or testimonial likeness may be used. It is also my understanding that I will receive no compensation for my likeness or testimonial.

Background Investigation

I authorize investigation (i.e. background checks) of all statements herein and release Camp Rising Sun and all others from liability in connection with the same. I hereby authorize Camp Rising Sun to verify any information provided on this application and release Camp Rising Sun and those providing such information, from any liability resulting from such inquiries. Additionally, I hereby authorize Camp Rising Sun to contact any person or entities I have had contact with or who has information about me, and further authorize any such person or entity to provide the organization with information, opinions, and impressions relating to my background or qualifications. I voluntarily release Camp Rising Sun and any such person or entity from liability involving the communications of information relating to my background or qualifications. I further authorize the organization to conduct a criminal background check if such a check is deemed necessary.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19/Flu/Strep
Camp Rising Sun has put in place numerous preventative measures and enhanced protocols to reduce the likelihood of spreading COVID-19 during our camp session. However, Camp Rising Sun cannot guarantee that you or your child will not become infected with COVID-19/Flu/Strep. Further, attending Camp Rising Sun could increase your child’s risk of contracting COVID-19/Flu/Strep.

By signing the agreement, I acknowledge the contagious nature of COVID-19/Flu/Strep and I assume the risk that my child, myself, and other family members may be exposed to or infected by COVID-19/Flu/Strep by while participating in activities at Camp Rising Sun, and that such exposure or infection may result in personal injury, illness, permanent disability or death.

I understand that the risk of becoming exposed to or infected by COVID-19/Flu/Strep at Camp Rising Sun may result from actions, omissions, or negligence of myself, my child and others, including, but not limited to, Camp Rising Sun volunteers, other campers and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injuries to my child, myself, and other family members (including, but not limited to, personal injury, disability or death), illnesses, damages, losses, claims, liability, costs or expenses, of any kind (collectively, “Claims”), that I, my child and our family may experience or incur in connection with my child’s attendance at Camp Rising Sun.

On my behalf, and on behalf of my child, I hereby release, covenant not to sue, discharge, and hold harmless Camp Rising Sun, its volunteers, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.

I understand and agree that this release includes any Claims based on the actions, omissions or negligence of Camp Rising Sun, its employees, volunteers, agents, and representatives, whether a COVID- 19/Flu/Strep infection occurs before, during or after participation at Camp Rising Sun.

Counselor Commitment Form

As a volunteer counselor with Camp Rising Sun, I understand what an incredible privilege it is to serve as a role model for our special campers.

In accordance with that understanding, I agree to protect the health and safety of the campers at all times while abiding by the following policies and procedures:
- Remember at all times that Camp Rising Sun is an endeavor which seeks to provide children with the opportunity to have a wonderful camping adventure filled with new experiences, lots of laughter, and avenues for building self confidence.
- Seek to exemplify the qualities of a person of Christian character (honesty, kindness, openness, fairness, patience, self-control, etc...)
- Seek opportunities to praise campers for their successes and/or to encourage them to persist until they are successful while carefully avoiding losing control of my temper.
- Refrain from any use of profanity, drugs, alcohol or tobaccos (including chewing tobacco and vapes) while a volunteer at Camp Rising Sun.
- Follow the rules governing Camp Rising Sun volunteers (a copy which will be provided upon acceptance), realizing and respecting that rules will be enforced with the overall goal that campers' safety and welfare is of utmost importance.
