I give permission for Pacific Link Housing:

•         to disclose my information to the Department of Human Services for the purposes of checking my account, billing or reference number, and amount I want to pay, and reconciling my payment Deduction details

•         to give the Department of Human Services my correct account, billing or reference number if required; and

•         to change my rental deduction from time to time to ensure my housing payments are met, not including arrears payments unless I provide new authorisation to do so.

I am aware:

•         that I have the right to change or cancel my deduction at any time, and

•         that further information about Centrepay can be found online at: servicesaustralia.gov.au/centrepaybusiness.

This agreement is between PLH and Tenant:

•         Where possible, I will reduce my amount owing / bond amount by making lump sum payments where possible, or by renegotiating the repayment amount per fortnight when my financial situation improves.

•         I understand that at least half of the Bond must be paid on signing my tenancy agreement.

•         Surplus funds in an account will be transferred between my accounts to maintain future account balances held with Pacific Link Housing.

•         Pacific Link Housing will issue rent statements to tenants at any time by request.

•         I also understand that if I fail to keep to this agreement, action may be taken to terminate my tenancy.
