Read the following important information before completing this form. Consistent bus stops are necessary for the safety of our students. Students are not allowed to make changes regarding pickup or drop-off stops. Example: riding the bus to a friend's house.

**You may only select ONE morning (AM) and ONE afternoon (PM) address for your transportation needs. Transportation will be set up within five (5) business days from the date this form is received in the Transportation Department.
