  • On a SUD scale, "1" is equivalent to feeling grounded, calm, and sensing your surroundings with comfort and ease. A "10" is equivalent to full panic mode, completely dissociated, and/or the worst distress you have ever experienced. 

    On the calibrated scale below:

    • First, you will describe the symptoms experienced mentally, physically, and/or emotionally. This may include what thoughts might be occuring in your head (Are you able to think in full sentences? Do you have "control" of your thoughts? Are you unable to form a complete, rational thought? Are your thoughts generally negative or positive?), what is happening in your body (Notice temperature, heartrate, breathing, tension, and/or other somatic symptoms such as nausea), and/or what you are experiencing emotionally (angry, sad, scared, annoyed, frustrated, paranoid, etc.). These symptoms may also relate to a recent event or situation.
      • For example, a level 4 may be "I feel nervous while driving. My heart is racing and I feel irritable. I am annoyed at the other drivers. I am having trouble concentrating or redirecting my thoughts."

    • Next, you will list what you need and what interventions can help (or what you hope will help) at each level.
      • For example, "I need to take a deep breath. Putting on my favorite music helps. Thinking of getting to see my cat when I get home. Or not looking at the time while I am driving."
  • Should be Empty:
