Enrolment Form- Yasmin Amber School of Dance

The following week after your child's first session you are required to complete this enrolment form. By doing so, you will be giving us the contact details we need in the rare case of an emergency and will provide us with the contact details needed to keep you updated with school activities.

You will also be required to read through and sign our dance school's policies, T&C's, payment agreement and photography policy all found later in this document to ensure smooth running of the dance school.

It is the parent/guardian's responsibility to keep the school updated (in writing) with any changes to vital information given in this form regarding their child's medical information, injuries, email address and emergency contact details etc.

CHILD PROTECTION:

To safeguard and protect our students, we ask for you to provide a password to use between you and YASOD concerning your child.

If you make arrangements for someone else other than Contact 1 or 2 to collect your child, we ask for you to inform the school principal prior to the arrangement. If we have your consent prior the password would not be needed.

In the case that someone unfamiliar to YASOD was to ask to collect your child without prior consent from the parent/guardian we would ask for them to provide the password below before we allow the student to leave our premises.

All children must be signed in and out of class by a Yasmin Amber staff member.

Children under 18 years must be collected by an adult from the village hall doors, unless we have written permission to say otherwise.
