The following week after your child's first session you are required to complete this enrolment form. By doing so, you will be giving us the contact details we need in the rare case of an emergency and will provide us with the contact details needed to keep you updated with school activities.
You will also be required to read through and sign our dance school's policies, T&C's, payment agreement and photography policy all found later in this document to ensure smooth running of the dance school.
It is the parent/guardian's responsibility to keep the school updated (in writing) with any changes to vital information given in this form regarding their child's medical information, injuries, email address and emergency contact details etc.
