Buggy Submission Form

By submitting this form, you agree to comply with the SLCGA Transportation Policy and you consent to SLCGA processing and storing the personal data contained herein for the purpose of administering competitions and processing your request to use a golf buggy.

No doctor/physio letter will be required to use a golf buggy in County competitions. We will rely on self certifications but the GB will verify with the home clubs that this is your normal way of playing at your club. This only applies to Suffolk competitions and not to regional

It will be valid for 1 year unless otherwised informed by the SLCGA GB. Please note that any expense incurred for completion of this form and submission to the SLCGA is the responsibility of the Player. The SLCGA have no responsibility for the availability or cost of a buggy and, where there are only a few buggies, it will be on a first come/first served basis.
