The Nicholasville Fabrication forms have been migrated to a new landing page and have new URLs associated with them. Many of you will not be impacted and will not notice a change as you are already using the new site, but some are still accessing by using the direct form links. The previous URLs have been updated with a re-direct link to the new hosting site, and will be fully deactivated on Monday, March 10th.

As a reminder, if your location is not listed in the Clinic Location dropdown menu then you are not setup to send that particular device type to the Nicholasville Fab.
