Thank you for becoming a patient with us! We are excited to work with you. In order to book your appointments, we first require acknowledgement of our Missed Appointment Policy. Once you have completed this form, you will be automatically redirected to our online scheduler.
If you do not find a time online that works for you, please call or text us at 206-448-9407 so we can schedule you.
Because we are a small appointment-based business, we enforce our 48-hour cancellation policy and require entering a credit card on file to book appointments online.
Missed Appointment Policy:
It is important that you make your appointments to get the best results from your treatment with us. We provide email and text reminders for appointments, however we still expect that you put the appointment on your calendar, in case our reminder system fails. We consider it your responsibility to know when your
appointment is scheduled.
An appointment is considered “missed” when it is canceled or rescheduled with less than 48 hours notice or if you do not show up for a scheduled appointment. We no longer waive the fee for COVID or other sickness because we still pay our providers when their time is reserved and there isn’t enough notice to schedule it with another patient.
In order to be reasonable, we charge only half of the cost of the service for a “missed” appointment. Our fees and terms are subject to change.
The fee for canceled (or no-show) appointments is as follows:
$50.00 for Massage Therapy
$75.00 for New Chiropractic Patient appointment
$30.00 for Follow up Chiropractic appointment
Missed appointment charges cannot be paid for by your insurance. If you miss more than 2 appointments without proper notice, or have any unpaid missed appointment fees, then we may cancel all future appointments.
We ask that you put a card securely on file below and then you will be redirected to our schedule. Your card is never charged as long as an appointment is cancelled or rescheduled with required notice.
