Purpose and Use of Recordings:

The audio-video photographs and recordings recorded in the event of Social Emotional Learning for Family using Breakthru Approach with 3PLAY may be used for promotional, educational, or informational purposes by Parents and Community Empowerment (PACE) Association. This includes, but is not limited to, website and book publication, social media sharing, newsletters, presentations, and fundraising campaigns.

 

Confidentiality and Privacy:

PACE Association will make reasonable efforts to ensure the confidentiality and privacy of all recorded material. However, it is important to note that once recordings are made public, PACE Association cannot guarantee complete control over their distribution or use. By signing this form, I acknowledge that I have been informed of this and release PACE Association from any liability related to the use and distribution of the recordings.

 

Testimonies and Interviews:

As part of the audio-video recording, PACE Association may conduct interviews or collect testimonies from me (parent) and my children (if applicable). These testimonies may be used in their entirety or in edited form for the purposes includes, but is not limited to, grant report submission, website and book publication, social media sharing, newsletters, presentations, and fundraising campaigns.

 

Consent for Children (if applicable):

As the parent or legal guardian of the child/children participating in the audio-video recording, I give my consent for my child/children to be recorded and for their testimonies to be collected. I confirm that I have the authority to grant this consent on behalf of my child/children.

 

Withdrawal of Consent:

If, at any time, I wish to withdraw my consent for the use of the audio-video recordings or testimonies, I understand that I must notify PACE Association in writing. However, I acknowledge that the recordings made before the withdrawal will remain in use as per the terms outlined in this consent form.

 

Agreement:

By signing this consent form, I confirm that I have read and understood the terms and conditions outlined above. I willingly give my consent for the audio-video recording of myself and my child/children and the collection of testimonies, as described in this form.
