Below, finish this form with a signature and a service deposit in the amount of $50. This amount will go towards the remaining balance paid upon arrival for your chosen service(s) and any add-ons or additional fees. In the event you need to cancel your appointment, a full refund of your deposit will be issued if canceled 24 hours or more ahead of the appointment time. If canceled between 24-12 hours before the appointment time, 50% will be issued, and less than 12 hours, no refund will be issued.
Photos and video of the vehicle will be documented before service to provide before and after photos to you, as well as for liability purposes. I am not responsible for any pre-existing damage, defects or otherwise, prior to performing service. I am not responsible for any mechanical issues you may have prior or after service, as I do not perform any work on mechanical parts or components.
