Appearance Release
The undersigned grants STSM creative and its designees and assignees the right to use my name, likeness, image, voice, appearance, whether recorded on or transferred to videotape, film, slides, photographs, digital media, digital recording, audio tapes, or other media. This grant includes without limitation the right to edit, mix, or duplicate and to use or re-use the Product in whole or in part as Producer may elect. Producer or its designee shall have complete ownership of the Product in which I appear, including copyright interests, and I acknowledge that I have no interest or ownership in the Product or the Products’ copyright. I confirm that I have the right to enter into thisAgreement, that I am not restricted by any commitments to third parties, and that New Life Church Creative Team has no financial commitment or obligations to me as a result of this Agreement. I hereby give all releases and clearances, copyright and otherwise, necessary for the use of my name, likeness, image, voice, appearance and performance embodied in the Product. I expressly release and indemnify Producer and its officers, employees, agents and designees from any and all claims whether known or unknown arising out of or in any way connected with the above granted uses and representations. The rights granted to Producer herein are perpetual and worldwide. I have read the foregoing and understands its terms and stipulations and agree to all of them.

Code of Conduct
Because STSM exists to teach, train, and send Holy Spirit empowered Christians into leading their homes, communities, cities and churches, we expect students to have a basic foundation of Christian behavior established in their lives prior to their initial enrollment. Students are required to be free from addictions or any other conduct inconsistent with the standards of STSM. I understand and qualify to meet the requirements explained above.

All applicants are required by South Texas School of Ministry to agree to a standard background check. STSM takes seriously our obligation to provide an atmosphere of safety for our students and visitors. We appreciate your willingness to comply and ask that you embrace with us the desire and the responsibility to provide this important level of safety. I hereby authorize STSM and its contracted third parties, to research character, general reputation, personal characteristics, and mode of living, as well as my driving record and/or criminal history, to be included in an applicant profile.
