Paws for Christ K-9 Training Enrollment Form

What you need to bring to class:

• Vaccination records from your vet (Rabies, Parvovirus, Distemper, Parainfluenza)
• 4' to 6' leather or nylon leash (no retractable or chain leashes)
• Buckle Collar, head halter, or harness
• A variety of small-bite treats
• Treat pouch or something to carry treats in
• A towel or dog bed

 

Terms & Conditions

I understand and agree that I am solely responsible for my pet and its behavior. If my pet hurts another person, pet, or property, I accept sole responsibility and will pay any and all related expenses, damages, and costs and hereby waive all liability against Paws for Christ K-9 Training. I understand there is an inherent risk in training, and there may be dogs of every breed, size, and temperament in the training class and where we will be training. I understand that my dog could come into contact with contagious diseases animal or human) and, though rare, we could be bitten, attacked, or seriously injured while participating in the dog training session. I understand that my dog may escape from the training area and be lost or injured. I expressly assume the risk of and accept full responsibility for any injuries and accidents that may occur due to my or my pet's participation in the dog training session and in any other activities connected with the course in which my pet participates. I hereby agree and understand that Paws for Christ K-9 Training and its employees are not liable for any injuries, damages, losses, costs, or expenses incurred by me or my pet as a result of my or my pet's participation in the pet training session and will look solely to the pet parent of the pet that caused any injury or damage for compensation to me or my pet. I understand Paws for Christ K-9 Training reserves the right to refuse or terminate services at any time if it determines, at its sole discretion, that I or my pet is not a suitable fit for the pet training session.

I understand that for the safety of all pets, proof of current vaccinations must be presented at the first class in order to participate.

Without expectation of compensation or other remuneration, now or in the future, I hereby give my consent to Paws for Christ K-9 Training, its affiliates, and agents to use my image and likeness and/or any interview statements from me in its publications, advertising or other media activities (including the internet).

This consent is given in perpetuity and does not require my prior approval.

If above named is under 18*, The below-signed parent or legal guardian of the above-named minor child hereby consents to and permits the above on behalf of such minor child.
