Individual Counseled remarks (Optional):
The leader summarizes the key points of the session and checks if the team member understands the plan of action. Once this statement is filled out, the team member will sign this statement to agree to all statements made on this document.
Once the leader fills out the statement and submits it, both parties will receive an email (usually within 5 minutes) for signatures. Both parties will sign the statement and will each receive an email copy. The leader must download the copy offline and archive it in the team member's file for future retrieval.
