YOU MAY EXPERIENCE SKIN SENSITIVITY/THINNING, WHICH CAN RESULT IN SKIN LIFTING, FROM THE FOLLOWING: SUNBURNED SKIN, RETINOL, CERTAIN MEDICAL CONDITIONS PREGNANCY, ANTIBIOTICS, OTHER MEDICATIONS NOT LISTED, MENSTRUATION

  • I understand the risks and dangers of waxing and agree to hold harmless Deluxe Wax Studio LLC and staff of all liability in relation to the service being provided and any future services. I agree if I begin use, or are currently using, any of the products listed in the above warning and do not inform Deluxe Wax Studio LLC prior to current or future treatments, I accept full responsibility for any adverse reactions. It is understood that waxing may cause some redness, bumps, soreness, and/or itching.

  • Eyebrow Tinting, Hybrid Gel Dye and Lamination Consent Form

    This form is required if you are getting eyebrow tinting, hybrid gel dye, or lamination. Before and after photos are required for this service for our records.

  • YOU CAN NOT GET EYEBROW LAMINATION, TINT OR HYBRID GEL DYE IF ANY OF THE FOLLOWING APPLY:

    Can not get an eyebrow lamination less than 6 weeks apart
    Sunburnt
    Are pregnant or breastfeeding
    Extremely dry or sensitive skin
    Have inflammation in the mucus membranes of the eye area
    Thin or sparse eyebrows
    Allergies to the ingredients used in brow lamination, tint or dye
    Cuts, abrasions, active acne or open wounds on the eyebrow area
    Skin conditions such as eczema, dermatitis, psoriasis, or rosacea
    Are using Retin-A, Accutane or other similar acne medications
    Have recently undergone a cosmetic procedure in the brow area, recent henna brows, or recent permanent makeup
    Have eye conditions or have recently had eye surgery


    Avoid using AHA/BHA products 7 days prior to your brow lamination service.

    Please do not wear contacts during your service.

    For tinting/hybrid gel dye: If you are concerned about allergies or have very sensitive skin. A patch test should be performed 48 hours prior to service. The product will be placed on the forearm. If no irritation or inflammation of the skin is apparent, it is usually assumed that no hypersensitivity to the tint exists.

    Although every precaution will be taken for my safety and well-being during eyebrow tinting, eyebrow dying, and lamination, I am aware of the following information and possible risks.

    I am aware some redness, irritation, itching, or bumps may occur on the skin where the lamination or tinting agent comes in contact.

    I understand I must keep my eyes closed and sit still while the esthetician is performing the service. 

    I understand an allergic reaction is possible

    I understand that it is imperative that i disclose all the information requested on the Client Consent Form.

    I understand that brow lamination is a process of reconstructing brow hairs to be styled in a desired shape, but it is my responsibility to brush my brows daily to maintain the desired look.

    I understand I need to wait 6-8 weeks between eyebrow lamination services.

    PORPER AFTERCARE FOR TINTING/DYE

    Do not wet the eyebrows for at least 24 hours after the treatment
    Avoid aggressive cosmetic treatment (scrubs, peels, alcohol-containing products) and steam
    (baths and saunas).
    Do not use lotions, toners, and creams in the eyebrow area.

    PROPER AFTERCARE FOR LAMINATION:

    Do not wet the eyebrows for at least 24 hours after the treatment.
    Avoid hot baths/showers, saunas, steam rooms, and swimming pools for 24 hours.
    Refrain from wearing eye makeup for 12 hours and use oil-free makeup remover when necessary.
    Avoid touching or rubbing the eye or eyebrow area to prevent irritation.
    Limit exposure to direct sunlight to avoid weakening the perm effect and fading the tint color.
    Avoid using chlorine and certain skincare products, as they may shorten the effects of the perm and tints.

    I have completed this form to the best of my knowledge, and I agree to inform my esthetician of any changes in the above information. I agree that I do not have any conditions that would make the requested treatment unsuitable. I will inform the esthetician of any discomfort I may experience at any time during my reatment to allow them to adjust accordilingly. I agree to waive all liability toward my esthetician and Deluxe Wax Studio LLC for any injury or damages due to any misrepresntatuion of my health. This agreement will remain in effect for this procedure and all future follow-ups conducted by the esthtician. I understand that this consent agreement is legal and binding. I have read and fully understand all the information in this agreement. By his/her signature below, he or she ratifies and consents to the procedure under these terms.

    Please read the policies below. This form is required before service. We truly appreciate your cooperation with our spa policies; they are necessary for protecting our business, time and ensuring the best possible experience for you.
  • Booking Policy

    To secure your reservation for our services, we kindly request a 25% deposit, which is credited towards your booking. Please be aware that this deposit is non-refundable. 

    • If you cancel with less than 12 hours notice, you forfeit the deposit, and a new one is required to book again.


    • If you cancel with more than 12 hours notice, you will be able to use the deposit towards your next reservation.

     

    Late Policy

    We kindly request that you show up on time for your reservation. There is a 7-minute late grace period. 

    • By 8 minutes past the appointment time, it will be cancelled. Please let us know if you will be late. 


    • If we are unable to accommodate you, then the reservation will be cancelled, you will forfeit your deposit, and you will have to reschedule to a different date. 

     

    Cancelation, Rescheduling, and No-Show Policy

    We understand if you need to move your reservation time or date. However, if you need to cancel, reschedule or modify your reservation, PLEASE GIVE US 12 HOURS NOTICE
    via text, (630-492-0890) email, or by going on deluxewaxstudio.glossgenius.com

    • SAME DAY CANCELATIONS WILL LOOSE THEIR 25% DEPOSIT IF YOU DID NOT SUBMIT A DEPOST: ZELLE THE CANCELATION FEE TO BE ABLE TO REBOOK.

    ( for example: a $60 service canceled on the same day would be subject to a $15 cancellation fee. )

    MUST BE PAID WITHIN 24 HOURS
    Or you will be blocked from booking future appointments.

    • This includes taking off services, if you know you will not need all the services you have booked, please notify us within the 12-hour time frame.

    • If you reschedule on Glossgenius, your deposit will automatically transfer over. (if canceled before the 12-hour mark.)

     

    Child and Extra Guests Policy

    We have room for 2 extra guests, who are able to sit still during your reservation.

    • Children must be accompanied by an adult at all times in the studio


    • Please keep children quiet and calm during the reservation to be mindful of other guests, and refrain from letting them touch any equipment or products in the suite. 

     

    Payment Policy

    Even if you are paying in cash, a 25% booking fee is required and goes towards the reservation. Please bring exact change if you are paying in cash.

    • We accept Visa, Amex, Mastercard, Discover, JCB, Union pay, Apple pay, Google pay and Zelle. Payment is to be made in full upon completion of all services. 


    • Please be advised that all sales are final on deposits, gift cards, services and products.

  • Age Verification and Parent/Guardian Consent (for guests under 18)

    If you're under 18 and want waxing services, we require a parent's or legal guardian's signed consent to comply with legal regulations. Please have them sign the form before your reservation. If you're over 18, simply input your birthday.
  • I, (parent/guardian) authorize Deluxe Wax Studio LLC to provide waxing services to (name of minor)

  • Facial and body waxing can be started whenever the minor and parent feel comfortable. Parent/guardian consent is required for any waxing for those who are under 18. The parent/guardian must accompany the minor for the first reservation, it is optional to be in the studio while service(s) are being performed.

  • Social Media Photo Release Form

    Photo consent to be featured on social media accounts. If you feel comfortable, please fill this out so we can create more content. This form is optional.
  • Authorization, Release, and Consent

    We would like to request your consent to take before and after photos of your waxing treatments at our spa. Before the treatment, we would take a photo of the area being waxed, and after the service, we would take another photo. We may also create short videos for use on our social media channels, such as Instagram Reels, to showcase the quality of our services. We assure you that all pictures or videos taken will be tasteful and respectful of your privacy. Any photos or videos depicting Bikini / Brazilian waxing will be censored to protect your modesty.
