  • Accreditation Renewal Applications are processed based on the date received and can take up to four weeks to process.

    Please allow four weeks for the accreditation to appear in your HockeyEd profile. 

    This will not affect your accreditation completion date.

    BEFORE SUBMITTING THIS FORM

    You must ensure the below steps have been completed.

    1. Contact your State or Territory Association to organise a practical assessment for your respective accreditation. A list of contacts can be found here: https://www.hockey.org.au/hockey-ed/hockeyed-contacts/

    2. Once you have completed your practical assessment, keep a copy of your signed and completed Practical Competency Checklist and record a digital copy of it (e.g., take a photo or scan the document). You will need this as evidence to submit with this form. 


    3. Complete your payment of $50.00 AUD, this is to cover administrative fees and the duration of your accreditation. Save a copy of your payment receipt. You will need this as evidence to submit with this form. Payments can be made here: https://www.hockey.org.au/product/32302/

    4. Your application will be reviewed, and if succesfull you will be notified via email and your accreditation will be updated in your HockeyEd account. 

  • In order to renew your Accreditation

    1. Users will need to ensure that they have created an account on the new HockeyEd platform. To get set up on the new platform, follow the instructions at the bottom of this page: https://www.hockey.org.au/hockey-ed/ 

    2. Once you have done this, be sure to take note of your new HockeyEd username. You can find this on the HockeyEd home page in the top left hand corner of the screen.
