Disclaimer:

The participants give consent to medical treatment in the event of emergency. NHMEF is not responsible for any loss and / or damage to any person and / or belongings whatsoever. The participants also agree that NHMEF will not provide any care or looking after anyone during the tenure of this trip. It will be travellers/participants own responsibility to look after their or their children’s welfare and well-being, buy adequate insurance to cover any loss etc. The participants give permission to the NHMEF for using their photographs featuring this trip for our publicity materials. The coach will leave for its destinations at the time specified on the top of this form. In case of anyone fail to get on the coach by the specified time, the coach will leave for its destination without the latecomers. NHMEF will not be responsible to transport the latecomers and they have to make their own arrangement to go/return to their destination. The information on this form will be solely for Forum's use, we will not pass the information to anyone else. We may contact you to inform of our other services which might be beneficial to you. We may hold this information for a particular period after which it will be destroyed. By signing this document, you are accepting all term and condition set out in this form and also giving us your consent to keep your information in our record and also to contact you in the future. You also agree that you have read NHMEF privacy notice (please see the page overleaf). You have the right to cancel this consent form anytime by giving us a written notice. To be signed by the travelling adults.

*Charges are strictly non-refundable unless we cancelled a trip.

On the day of the trip, please report to an NHMEF official(s) on arrival. They will confirm your attendance and allocate your seats. You must not board the coach before the official (s) authorises you and your party members to do so. Anyone who is in breach of this may be refused travel without refund.

The information you provide (personal information such as name, address, email, phone no) will be processed and stored so that it is possible to contact you, provide information and/or access our facilities and services. Your personal information won’t be shared or provided to other 3rd party.

NHMEF ensures the security of personal data or information is protected from unauthorised access, loss, manipulation, falsification, destruction or unauthorised disclosure. This is done through appropriate technical measures and appropriate policies. We will only keep your data for the purpose it was collected for and only for as long as is necessary. After which it will be deleted.

Children

We will not process any data relating to a child (under 13) without express parental/ guardian consent of the child concerned.

NHMEF does not use automated decision making or profiling of individual personal data.

In accordance with the law, we only collect a limited amount of information about you that is necessary for correspondence, information and service provision. We do not use profiling; we do not sell or pass your data to third parties. We do not use your data for purposes other than those specified. We make sure your data is stored securely. We delete all information deemed to be no longer necessary. We constantly review our Privacy Policies to keep it up to date in protecting your data.
