Ages: 6th grade to Adult (will accept younger youth based on maturity)

Date: July 24th-28th

Time: 8:45 am-1:15 pm

By signing below you give the above listed children permission to attend VBS at Andrew Chapel United Methodist Church. You authorize any treatment by an accredited hospital or physician deemed necessary in case of an emergency and you agree to not hold Andrew Chapel United Methodist Church, its leaders, employees, volunteer, and/or staff, liable for damage, losses, diseases, or injuries incurred.

 

You also give permission for you and/or your child to be photographed at VBS by Andrew Chapel United Methodist Church volunteers/staff for the express purposes of in-house advertising on the church website, social media, and in-person worship services. If you do NOT wish for your child to be photographed, please type NO in the permission box.
