TSI/Accuplacer Testing Request Form

Before submitting this form, please be sure to complete the following steps.
- Apply for Admissions.
- Complete the Pre-Assessment Activity (PAA) using this link: Online PAA. You will receive a certificate of completion when you are done via email, which you must present on the day of the exam. You can email this to us if you are testing remotely.
- Contact Admissions or Advising FIRST to find out which test sections you need. You can also find out information about exemptions on the Placement Testing website.
- Pay for your exam online. You can pay in person if you plan to test on campus.
- If testing in person, make an appointment, which is required. No walk-ins will be allowed.
- If testing remotely from home, wait for an email from Accuplacer and North Harris Assessment Center with your remote voucher number and instructions on how to schedule with the 3rd party company, Examity. Please allow 1-3 business days for processing.
- If you are testing for Dual Credit AND this is your first time, please see your high school Counselor and DO NOT submit this form!
- If you are re-testing for Dual Credit, please proceed with this form.
  
  NOTE: Examity will charge a $25 fee for their proctoring services. You will pay this fee directly to them, not Lone Star College. This is in addition to the testing fees paid to Lone Star College.
