Liability Waiver:
I as an athlete or parent of athlete understand that sports activities will include play indoor and outdoor sports activities. I also understand that outdoor activities may occur in the hot sun and in the rain. I agree to see that all participants are appropriately attired for sports activities, I do not expect DYPL Management to provide these items.
DYPL is a property of Mustangs Cricket Academy, and any of its affiliates, members, coaches, agents, employees, officers, directors, contractors, sponsors, and other representatives, successors and assigns (collectively, the “Released Parties”) are not assuming any responsibility of any kind. Participant assumes full risk and responsibility for any injury - minor or major, disability, death or bodily injury that my kids as participants may suffer, or damage to property that may be caused, arising from any cause whatsoever, or any death, disability, or bodily injury or property damage to another person arising from Participant’s participation in the Event. Participant forever discharges and waives any claim or legal action of any nature, known, or unknown, in law or in equity, which Participant or Participant’s heirs or legal representatives shall or have made in that regard.
The above-mentioned participant also understands that there is a risk of loss, theft, and damage to any personal belongings, personal equipment, and other property that Participant brings onto the DYPL tournament, and the Participant does not and will not hold the Released Parties responsible in any way.
In consideration of the acceptance of participation in the DYPL tournament, Participant, directly and on behalf of Participant’s heirs and legal representatives, releases and forever discharges the Released Parties from any and all claims, damages, losses, or injuries that Participant may suffer or sustain, by negligence or otherwise, arising out of or relating to Participant’s participation in the Event, including with regard to any travel to and from the event. All such claims, whether known or unknown, are hereby waived and released and Participant covenants not to sue thereon.
Medical Release:
I hereby consent to the rendering of emergency first aid or medical treatment deemed reasonably necessary for Participant by the Released Parties at the time of injury or illness in their sole discretion. I further understand that I will be responsible for payment for any such medical treatment if required. I am fully aware of the risks involved with entering the DYPL tournament assigned cricket fields, as well as any transportation and/or during the COVID-19 pandemic or similar infection related, I am entering into this waiver knowingly and on my own behalf
Concussion Waiver.
While DYPL Tournament involve minimal contact and the risk for concussions in our tournament is low, we ask that you read and acknowledge that you understand our concussion safety policy below.
What is a Concussion?
A concussion is a type of traumatic brain injury—or TBI—caused by a bump, blow, or jolt to the head or by a hit to the body that causes the head and brain to move rapidly back and forth. This sudden movement can cause the brain to bounce around or twist in the skull, creating chemical changes in the brain and sometimes stretching and damaging brain cells.
The CDC estimates as many as 3.8 million concussions occur in the U.S. annually through sports and recreational activities.
The Facts Recognizing a Possible Concussion
1. A concussion is a brain injury.
2. All concussions are serious.
3. Concussions can occur without loss of consciousness.
4. Concussions can occur in any sport.
5. Recognition and proper management of concussions when they first occur can help prevent further injury or even death. To help recognize a concussion, you should watch for the following two things among your athletes:
1. A bump, blow, or jolt to the head or body that results in rapid movement of the head.
2. Any change in the athlete’s social or emotional behavior, thinking or remembering, physical or sleep functioning. You may notice signs before the athlete does.

“When In Doubt, Sit Them Out” Policy:
An athlete who is suspected of sustaining a concussion or head injury will be removed from the practice or game immediately and will not be allowed to return to play without written clearance from a licensed health care provider. The player should not be left alone, and the parent/guardian will be notified to follow up accordingly.
BY REGISTERING, I ACKNOWLEDGE THAT I HAVE READ AND FULLY UNDERSTOOD ALL OF THE TERMS OF THIS AGREEMENT AND THAT I AM VOLUNTARILY GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE COMPANY, WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME. I INTEND MY SIGNATURE TO BE THE REQUIRED EVIDENCE OF MY ASSENT TO COMPLETELY AND UNCONDITIONALLY RELEASE ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
I also agree to allow their photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers and assigns. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. The DYPL Management, its Trustees, directors, officers, and all its employees, acting officially or otherwise are hereby released from any and all claims, demands, actions, or causes of action on account of any injury to my child that may occur. This release binds my heirs, executors, administrators, and/or assigns.
Registration Agreement:
As a parent/legal guardian to the participant and in consideration of being allowed to participate in any way in the DYPL Tournament, I agree and understand the following:
The registration fees are non-refundable under all circumstances except if the athlete is not picked in the draft. There are no guarantees to the number of games or amount of time the participant will play.
I understand that the chief goal of DYPL Management is for me and my child to have an outstanding experience. If it does not deliver the Experience as outlined when I registered, I will notify the Tournament commissioner of my issue/concern in writing as soon as possible so that he/she may have the opportunity to satisfy me.
I further understand that DYPL Management will not reserve a place for any child on a schedule or team until full payment has been made.
I will follow all rules, regulations, and bylaws set by DYPL Management committee, and their respective assigns, licensees, and legal representatives.
I have read this registration agreement and fully understand its terms. I also understand that I have given up certain rights by signing it, as described above, which I agree to do voluntarily.
I am the parent or legal guardian of the minor named on the registration form. I have the legal capacity to consent to and, by registering, I hereby do consent to the terms and conditions of this Release and Waiver of Liability and Assumption of Risk on behalf of my child or ward.
