This form is for the anonymous reporting of Code of Conduct violations at the International HPC Summer School (https://www.ihpcss.org/). You can view the code of conduct on the event website. No identifying information is required. If you choose not to provide an email contact, then the COC committee will have no way to inform you of any outcome of this report. The contents of this report will be provided to all the members of the COC committee (as detailed on the COC itself).
All IHPCSS attendees will be provided a basic summary of COC reports at the close of the summer school.
