  • Your counselor is a Level I certified biblical counselor and is now in the process of obtaining a Level II certification in biblical counseling through the Association of Biblical Counselors (ABC) via So That We Biblical Counseling, LLC. He/she is not licensed by the state as a professional counselor.  So That We Biblical Counseling only supervises interns with a minimum of a bachelor's degree and preference for a master's degree or master's in progress.  Your counselor's credentials are listed on his/her bio on the website of So That We Biblical Counseling, LLC.  The course in which your counselor is enrolled is an advanced course training him/her advanced concepts in how to utilize biblical truth in the context of discipleship and counseling. This Level II Advanced Training implies that he/she has already learned the basic skills in this area and is now learning advanced skills.

     

    By signing this form you:

     

    • Understand that your counselor has earned Level I certification through ABC
    • Understand that you are engaging in an advanced training process (Level II) with your counselor
    • Understand that your counselor is supervised by lead counselor, Krista Ryndak, and you have the right to contact her at any time with input, questions, or concerns about the care your counselor is providing
    • Understand that you have the same right to confidentiality within So That We Biblical Counseling, LLC as with a non-intern counselor, as outlined in the Client Intake Form

     

    Supervision primarily takes place by case study review.  This means your intern counselor discusses your case with the supervisor and the supervisor makes suggestions.   However, each counselor is required to have a certain number of  hours of direct supervision, meaning the supervisor directly observes them as they counsel, in order to make more specific suggestions to assist in their growth.  This can be by recording or in-person observation.  By utilizing an intern counselor, YOU ARE NOT REQUIRED to consent to either of these options.  If you are willing to participate in direct supervision sessions, please initial your preferences below.  If not, please skip to the bottom of this form.

  • Consent to record:

     

    As part of the counselor's required coursework, his/her sessions will, on occasion, be recorded and listened to/viewed by the supervising lead counselor only. These sessions are deleted after viewing and are kept confidential between your counselor and the supervisor. Please initial below if you consent to have your session recorded for the counselor's training purposes only.

  • Consent to attend:

     

    If you do not wish to have sessions recorded but are willing to have the lead counselor present in-person to attend one of your sessions, please initial below.  The lead counselor will not lead the session and will only attend for purposes of observing the intern counselor for their training and skill growth.

  • I am willing to enter this advanced training experience with the cited counselor and understand the intent and purpose of this process. Assignments based on our time together may be turned in to the supervising counselor. However, I understand my name will NOT appear on any assignments and that all content on this form will be held confidentially by ABC and So That We Biblical Counseling, LLC.
