This form is designed to give information needed to make an informed choice of whether or not to undergo a permanent cosmetics/tattoo application. If you have questions, please don’t hesitate to ask.
Although tattooing is effective in most cases, no guarantee can be made that a specific client will benefit from the procedure. This is the process of inserting pigment into the dermal layer of the skin and is a form of tattooing.
Notice: tattoo inks, dyes, and pigments have not been approved by the federal Food and Drug Administration and that the health consequences of using these products are unknown.
All instruments that enter the skin or come in contact with body fluids are in a sealed/sterilized package before use and disposed of after use. Cross contamination guidelines are strictly adhered to.
Generally, the results are excellent. However, a perfect result is not a realistic expectation. It is usual to expect a touch-up after the healing is completed.
This procedure is done by hand, which indicates there is room for error and small discrepancies. This is normal for any cosmetic procedure. Initially the color will appear much more vibrant or darker compared to the end result. Usually within 60 days the color will fade 10-50%, and may look softer and/or more natural for cosmetic procedures. Body art tattooing can also fade or shift in tone, lines may expand, thinner lines may "fall out". Touch-ups are needed to maintain the shape and color.
Tattoo results are not guaranteed. If you are not happy with the results no litigation or social media slander can be taken against Lydia Lockheart PMU/Lockheart Tattoo or Lydia Henry. You will not submit a "chargeback" or dispute charges thereafter completing your appointment.
