Thank you for your interest in availing of our services. At Innovatons Salon, we take pride in providing the best hair color service to the best of our capacity. Please take note that we reserve the right to decline treatment in case we believe that the hair is not healthy enough for treatment.

I hereby acknowledge that there are risks involved in any hair treatment and use of cosmetic products that may lead to allergic reactions or chemical reactions which may lead to skin irritation, discomfort, or illness.

I hereby release with full knowledge and voluntary consent, Innovations Salon, its employees, owners, directors, affiliates, the management, and partners from any and all liability, damages, claims, and causes of action arising from any harm, injury, discomfort, damage, or death. Further, I hereby declare that I have no illness or any condition that might give adverse effects from the procedure I will undergo. I have been informed of the necessary precautions that I need to comply with. Innovations Salon cannot guarantee my color I understand without using the products recommended.

If you are having corrective color this means:
● More time is required than with a standard color service.
● Reconstructive treatments may be needed prior or post to services.
● Multiple visits will likely be required to achieve your desired color.
● Each subsequent visit will be a separate color service with separate charges applied.
● Specific products/treatments may be recommended to use at home to help guarantee colorcorrection results.

I declare that I am of legal age or represented by a parent/guardian with full competency to sign this consent. I voluntarily agree in accepting the terms and conditions for the services to be performed and I am executing this document voluntarily without any representation or coercion.

Signing below indicates your consent and agreement to indemnify, defend and hold harmless Innovation Salon, it’s owners, employees, agents and assigns from any liability claim or action arising from the application of hair coloring products.
