IMPORTANT LASH LIFT TREATMENT DO'S & DONT'S

To assure long lasting results, please thoroughly read the information below.
PRE-CARE INSTRUCTIONS
1. Avoid using waterproof mascara 2-3 days prior as it can leave a residue that may prevent lashes from properly lifting.
2. Come to your appointment without any eye makeup or product.
3. Ensure that you do not have an eye infection or skin irritation of the brow and eye area - as treatment cannot be performed until this has cleared up.
AFTER-CARE INSTRUCTIONS
1. Avoid hot showers, steam & sauna for 24-48 hours.
2. No sweating & keep treated area dry for 24-48 hours.
3. Avoid prolonged sun exposure for 24-48 hours.
4. Avoiding rubbing or touching of the treated area for 24-48 hours.
5. Avoid applying make up (mascara, serums, etc) on the area around the brow for 24-48 hours.
6. Sleep on your back.
7. Brush daily & start castrol oil (provided in aftercare kit) on day 2 to condition, hydrate and keep your natural hairs healthly.

By checking off each item below, I am informing my technician I fully read and understand the following in order to receive my scheduled service for a lash lift and/or, tint to my natural eyelashes.

I consent to having my eyes closed for the duration of the 45-90 minutes for the service I scheduled.I agree to have my lashes lifted and/or tint applied to my natural lash hairs by Skin Essentials Studio.I understand there are risks associated with this lash service. I further understand that as part of the procedure, skin irritation, itching, discomfort, and in rare cases eye infection, inflammation or blurriness could occur. I agree that if I experience any of these medical conditions with my eyes, skin area around the eyes that I will contact Skin Essentials Studio immediately and consult a physician at my own expense.I understand that even though my technician perms/lifts, my eyes, using the proper technique, the sanitized instruments, tapes, cleaners, eye gel pads, adhesives, and removers used may irritate my eyes/skin or require a physician’s follow-up care.I understand and agree to the care instructions provided by my technician for the use and care of my lifted and/or tinted natural lashes. I realize and accept the consequences of failure to adhere to these instructions may cause the tint and natural hairs to not stay permed as long as told.I understand that it's my responsibility to request for a patch test 48 hours before treatment.I consent for staff to take photos and videos of me during my service. I understand and allow Skin Essentials Studio to use photos and/or videos or me to be posted online or on social media for educational, marketing and business purposes.My completed eye lash treatment consent form is valid for 1 year from this submittal date. I will inform Skin Essentials of any changes to my health, skin and eyes to continue treatments.
BOOKING POLICY

By checking off each item below, I am informing my technician I fully read and understand the following in order to receive my scheduled service for brow lamination, tint to my brow hairs and skin and/or wax.

GRACE PERIOD - If I'm running 10+ minutes late, I understand appointment is considered a "No Show" and I will be charged 100% of your booked service(s) unless I'm told otherwise.RESCHEDULE or CANCELLATION - Recognizing that everyone's time is valuable and the appointment time is limited, I understand I need to provide a 48 hour notice if I'm unable to keep my appointment date and time. Rescheduling or cancellation within 48-hours of my appointment will result in a cancellation fee.NO SHOW - "No Show" fee of the full appointment cost will be charged if I cancel my appointment day of or fail to show for an appointment. This fee cannot be put towards a future service, and will be charged to the credit card on file at the missed appointment date and time. If I reach three no shows in a row, I risk being terminated from booking services with Skin Essentials. (For ClassPass Clients, you’ll be fully charged thru CP with your booked credits.)

Acknowledgement and Waiver

I am over 18 years of age and consent to the agreement and to the treatment, or have a parent with me that consents to this service. This agreement will remain in effect for this procedure and all future procedures conducted by Skin Essentials Studio. I understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I release Skin Essentials Studio from all liability associated with the above treatments, which is performed with the utmost attention to safety and proper application using tools and products that the technician has been trained to use. There are no guarantees for length of time the brows will stay lifted and/or tinted.

I understand the aftercare instructions and will do my part to maintain my brow lamination and tint. I understand that there are many factors that may affect the life of the eye brow lamination lift and/or such as water and moisture contact, weather conditions, and activities involving exposure to high temperatures.
