  • Camper Check-Out on Friday

For the security of your child, we will be asking to see your driver’s license at check-out.
  • If you are unable to pick your camper up at the end of camp and someone else will be providing transportation for her, we need your permission in order to release your camper to them.

  • Internet Permission Slip

  • As a STEM camp we try to incorporate as much technology as possible.  While your daughter is at camp we will be taking photos/videos of every activity she participates in.  The photos/videos are a way for the family to feel connected while their daughter is at camp.

What photos/videos and how will they be used:

    • Throughout each day some of the photos/videos are used to post on Instagram and TikTok.
    • Each evening some photos are used for the nightly parent email.
    • Last day of camp we will take all camp photos, and family photos.
    • At the end of the week all photos/videos will be dropped into google photos.  Parents can purchase all of the photos for $10.  A link will be emailed out after camp ends.
    • Some photos will also be used on our website and app to help encourage more girls to attend the Be WISE Camp.

In order for us to be able to use these photos we need the parents to sign the permission slip below.

  • Scholarship

This should only be completed if you filled out an application for a scholarship with another organization other than Be WISE Camp. This is NOT a scholarship form, it is just so we know who to contact about the scholarship you were offered or applied for.
  • Thank you for completing the Be WISE camp application. The teacher recommendation form will automatically be sent to the teacher you recorded.

The potential camper will receive an email with a personality assessment.  This needs to be compelted for rooming purposes.

After the teacher recommendation has been submitted, your daughter will either be approved for camp, or denied. You will receive a notification through email from Jotform.

When approved, you will then need to complete the other forms sent from jotform.

FYI...our camp has a no cell phone policy.  We want to make sure that the girls are embracing the entire camp experience and they are not distracted by the phone.
