The mailing address cannot be changed once submitted. We cannot send the urn and death certificates to different addresses. This agreement overrides any previous communication with our staff. Charges are only for those items that you selected or that are required. If we are required by law or by a cemetery or crematory to use any items, we will explain the reasons in writing here: A rigid, combustible container is required by the crematory. A cremation permit is required by state law.

If you selected an arrangement that may require embalming, such as an arrangement with viewing, you may have to pay for embalming. You do not have to pay for embalming you did not approve if you selected arrangements such as a direct cremation or immediate burial. If we charged for embalming, we will explain why here: none.

We charge you for our services in obtaining: death certificates four dollar charge for each. Five percent credit card or debit card recovery fee. One hundred fifty seven dollar cremation permit required by the state for cremation. We reserve the right to modify the credit card/debit card recovery fee as needed. By submitting you agree to charge your card for the above amount and you agree to our electronic signature terms of service and refund policy. Refund policy: Because of the sensitive nature of the services we provide we are unable to offer refunds.
