INSTRUCTIONS:

• Read and sign the GLC Life App forms submitted by your members. Forms for each level are available on the GLC Website (https://glc.ccf.org.ph/events/glc-graduation) or at the GLC Booth. Once signed, return the Life Apps to your members.

• Instruct your members to submit their Life Apps through this link: LifeApp Submission Form. After they submit, a reference number will be generated. Ask them for this number and record it in this Form (LA Ref No.).

• Collate all your members’ Life Apps.

• Write a summary for each of your recommended members. Highlight inspiring testimonies of transformation and demonstrated growth in their discipleship journey throughout the GLC training.

• Submit this form along with all the completed Life Apps from your members. Include your own Life App if you completed the training with them.

The information collected and stored will be used solely for the purpose of GLC which is not limited to check in and registration during related event and activities. Changes will only be made upon collection of updates by GLC and/or the request of the participant. Any and all disposal of data will be carried out securely and within the timeframe provided by CCF. Information stored in a Third Party Database System is subject to the Privacy Policy of CCF. The participant waives any and all liability upon the disclosure of personal data to CCF.
