Hockey SA Juniors Playing Seniors Consent Form
Pursuant to Rule 10.7.3 – Juniors under the age of 15 must seek permission from Hockey SA prior to playing in the senior competition via the permit process at Rule 11. Inclusive in this permit application needs to be parent/guardian consent to the junior playing within specific senior grades.

Parents/Guardians of juniors wishing to participate in the 2024 Hockey SA Men’s and Women’s Senior Competition are requested to read and sign this form giving consent for their child to play prior to taking to the pitch.

This consent expires at the completion of the 2024 Hockey SA Senior Metropolitan Competition.

I understand that it is at the discretion of Hockey SA to deny entry of a junior player into the senior competition, if considered not suitable or appropriate.
