  • This form is to be completed by NHS organisations to inform the Academy of any changes in circumstances of a sponsored MTI doctor. These include:

    • does not arrive, or is not expected to arrive, in the UK on the agreed date;

    • does not commence employment on the expected start date;

    • moves to a different role within the same organisation
      (if the doctor transfers to a different organisation, please complete the Transfer/Extension Form instead);

    • experiences any change to their salary or contracted working hours.

    This form should also be completed if you become aware that a sponsored MTI doctor is:

    • continuing to work for the same organisation but at a different work location;
    • in breach, or suspected breach, of the conditions of their immigration permission; or
    • suspected of engaging in criminal activity or other conduct that may contravene their visa conditions.

    Please give all names EXACTLY as they appear in the passport
  • Please ensure that the MTI doctor’s name is entered exactly as it appears on their passport, as this information is reported directly to UKVI. Any missing or incorrect name details may delay our ability to identify the doctor and complete the reporting process.


  • Please note: If a medical Royal College or other organisation was involved in the doctor's placement in any way and they are not listed on this report, we will reject the report.
