Medical Training Initiative
This form is to be completed by NHS organisations to inform the Academy of any changes in circumstances of a sponsored MTI doctor. These include:
- does not arrive, or is not expected to arrive, in the UK on the agreed date;
- does not commence employment on the expected start date;
- moves to a different role within the same organisation
  (if the doctor transfers to a different organisation, please complete the Transfer/Extension Form instead);
- experiences any change to their salary or contracted working hours.
This form should also be completed if you become aware that a sponsored MTI doctor is:
- continuing to work for the same organisation but at a different work location;
- in breach, or suspected breach, of the conditions of their immigration permission; or
- suspected of engaging in criminal activity or other conduct that may contravene their visa conditions.
