    Medical Training Initiative
  • This form is to be completed by NHS organisations to inform the Academy of any changes in circumstances of a sponsored MTI doctor. These include:

    • When a sponsored MTI doctor will not arrive/has not arrived in the UK on the expected date
    • When a sponsored MTI doctor has not started work on the expected date
    • When a sponsored MTI doctor has moved to a different role within the same organisation. If the doctor has moved to a different organisation, please complete the Transfer/Extension form.
    • When there are any changes to a sponsored doctor's salary or work hours.

    This form should also be completed if you have information that an MTI doctor is:

    • Working for the same organisation but the location at which the doctor is employed has been changed
    • Breaching the terms or conditions of his or her leave
    • Possibly engaging in criminal activity or other activities in contravention of their visa conditions
  • Part 1: Details of the MTI doctor and post

    Please give all names EXACTLY as they appear in the passport
  • Please ensure the MTI doctor's name is included EXACTLY as shown on the Passport as this information is reported directly to UKVI.

    Due to the large amount of doctors on the MTI scheme, missing any part of the doctor's name can make it impossible to determine who the report is for. 

    We will reject any report that we cannot conclusively link to a specific doctor.


  • Please note: If a medical Royal College or other organisation was involved in the doctor's placement in any way and they are not listed on this report, we will reject the report.

  • If you are reporting a change in salary for a "home funded" MTI doctor, you must provide evidence of fundsing to support the new salary level.
