SignatureMo’ Jam Presents LLC and its representatives record audio and film every event of “ Mo’ Jam Mondays”. We hereby request you review and agree to the following terms:1. I hereby release Mo’ Jam Presents LLC and its representatives from any claims, demands, actions, suits, or liabilities, whether in law or in equity, arising out of the use or distribution of recorded being audio or video material.2. I acknowledge that my participation is voluntary and that I will not receive any monetary compensation or royalties for my appearance, image, or likeness in the documentary.3. I consent to the use, reproduction, distribution, and display of the recorded material, including but not limited to television broadcasts, streaming, online platforms, film festivals, and promotional materials related to the documentary.4. I confirm that I am of legal age and have the right to enter into this agreement, or I have obtained the necessary consent from my legal guardian to do so.5. I understand that the rights granted in this release are perpetual and worldwide.6. I understand that Mo’ Jam Presents LLC will make reasonable efforts to protect my personal information, but I acknowledge that my image and likeness may be seen by a wide audience and that I have no control over how the recorded material may be used by third parties.7. I have carefully read and understood this release and consent form and agree to be bound by its terms.
