To ensure the success of the commissioning visit, please complete and submit this form for each pumping station that requires commissioning. The commissioning date will be confirmed once we receive the completed form, please note that 5 working days notice is required for commissioning prior to our engineers attending, please ensure the following checklist has been accurately completed.

Ensuring the accuracy of this form completion is critical to avoid unnecessary delays and additional costs.

If any of the following information is incomplete or inaccurate on attendance to site, this will result in an aborted visit charge of £650.00 + VAT.

WORK TO BE UNDERTAKEN BY PARADIGM

Install pumps by lowering into the tank, and set float switches at the required depth.
Pull extra cable (if agreed in writing with the client)
Pull pump & float switch cables through the cable duct, and connect to the control panel.
When power has been connected to the control panel, test and commission the installation.

If any additional items are required, please notify Paradigm as soon as possible, to reduce any delays or costs arising from the additional items.

No confined space works or working at height is required to commission this pumping station.
