- Removed the limitation on the End of Placement date to allow any date to be entered. This will alow us to gather all the required information in time to withdraw our sponsorship on the MTI doctor's final working date
- Added updates section to ensure transparency with the changes made to this form

This form is to be completed by the NHS organisation employing the MTI doctor.
An End of Placement Report must be submitted when the MTI participant is in one of the following circumstances:
- Has reached the end of their placement
- Has resigned
- Has been dismissed
Please note: and End of Placement report must NOT be submitted unless one of the above conditions applies. Specifically, it must not be submitted if the doctor is transferring from your organisation to another one, otherwise their sponsorship will be cancelled, leaving them in the UK illegally.
