- End of Placement Report – Important Guidance
  To be completed by the NHS organisation employing the MTI doctor
  
  This report constitutes a formal request to end the Academy’s visa sponsorship.
  It must only be submitted if the doctor:
  - has completed their placement;
  - has resigned from their post; or
  - has been dismissed.
  Do not submit this report if the doctor is transferring to another UK organisation. Submission in these circumstances may:
  - result in the cancellation of the doctor’s visa sponsorship; and
  - leave the doctor without valid immigration status in the UK.
    Please ensure that the above conditions are met before submitting this report.
  If you are unsure whether this report should be completed, please contact us for guidance.
