End of Placement Report – Important Guidance
To be completed by the NHS organisation employing the MTI doctor

This report is a formal request to terminate the Academy’s visa sponsorship.
It must only be submitted when the doctor:
- Has completed their placement
- Has resigned
- Has been dismissed
  
  Do NOT submit this form if the doctor is transferring to another UK organisation. Doing so will:
- Cancel the doctor’s visa sponsorship
- May leave them without valid immigration status in the UK
  
  Please ensure these conditions are met before submitting the report. If you're unsure, contact us for clarification.
